It is the policy of Targeted Therapeutic Massage that all brand new clients requesting an appointment must first have a phone consultation before the initial appointment will be accepted. Intake forms will then be sent by email. Please check your junk mail folder for them if you can't find them. You are responsible for reaching out if there is an issue receiving your forms. Intake forms must be COMPLETE and FULLY FILLED OUT. Incomplete forms will not be accepted and a request will be given to complete any blank or incomplete sections before appointment is accepted. Targeted Therapeutic Massage will not be making any guesses or assumptions to the answers or responses to any parts of your forms no matter how trivial the question or response may seem.
All forms must be completed and returned at least 24 hours prior to your appointment.
Exceptions may possibly be made on a case by case basis at the sole discretion of the therapist, however in the event that forms are filled out on arrival, or not received in advance, any time spent to review the forms or fill them out will be part of your session time. Session length or the appointment time will not be prolonged to make up for that time. If an exception is not made and forms are not received at least 24 hours prior to your appointment, your appointment may be declined or rescheduled. It is solely your responsibility to make sure forms are submitted on time, and this should be taken into account when booking an appointment less than 48 hours in advance.
